Daycare & Boarding Forms
Enrollment Form
Our Enrollment Form is the first step in getting your pet ready for boarding or daycare! It helps us gather important contact information.
Please fill it out and submit via email or by sending a picture to our phone number before your pet’s first visit.
To enroll multiple pets, use the additional Guest Information boxes located on Page 2 of the downloadable form.
Pet Behavior Form
Understanding your pet’s unique personality helps us provide the best care possible. Our Pet Behavior Form allows us to learn about your dog or cat’s likes, dislikes, energy level, social preferences, and any behavioral quirks or triggers.
By gathering this information ahead of time, we can ensure a more comfortable, safe, and personalized experience—whether your pet is here for daycare or boarding.
General Policies
To ensure the safety of both pets and people—and to be transparent about our expectations—all clients are asked to review and complete our General Policies. Whether your pet is visiting for daycare, boarding, or grooming, this form covers essential information, including our procedures, safety guidelines, vaccination requirements, and emergency contacts.
Having clear policies in place allows us to deliver consistent, high-quality care for every dog and cat that comes through our doors!
Daycare & Boarding Policies
Our Daycare and Boarding Policies are designed to make you aware of things that may happen while your pet is in our care. Please take time to read this carefully and ask any questions before submitting it to our team before or at your pet's first visit.
If you are a family with multiple pets, only one form needs to be filled out.
Check-In Form
Boarding ONLY
When your pet(s) is/are coming to stay with us, the Check-In Form is required to be filled out for each pet.
Be specific with your arrival and departure times to help us ensure everything is ready and organized for your pet(s). It also allows us to avoid multiple pets arriving or leaving at the same time, which helps keep everyone safe and calm.
Page 2 includes optional add-ons to customize your pets boarding experience. If you'd like your pet(s) groomed during their stay, please schedule the appointment in advance to ensure availability and can plan accordingly.
